Distro: Arch GNU/Linux DE: KDE 4.7 RC1 Plasma Theme: Glassified Widget Style: QtCurve Icons: Hycons
I recommend it to be used with 'plasma-widget-menubar' or 'bespin's xbar' or 'oxygen-appmenu', the menubar background perfectly matches with the window background gradient though.
Tip: System Settings> Workspace Appearance> Window Decoration> Oxygen> Configure Decoration> Fine Tuning> Background Style = Solid Color
Oxygen (the windeco) has been improved in KDE 4.7 and got some new intresting features
Download (save) the image for a sharper preview, it's kind of blurry here on dA.

I do not know which distro you are using, but in general, search for 'qtcurve' in your distro's repos and install whatever package which 'qtcurve' is a part of its name! Then go to 'System Settings> Application Appearance> Styles', select 'QtCurve' from 'Widget style', click on 'Configure' and import the qtcurve config file. After that go to 'Colors' and import the color scheme file. That's about it

I don't know which distro you're using, it's best to ask this question in the dedicated forums of your distro. In general, you need to install QtCurve and Smaragd first, search for how to use them or ask in forums. If you had any specific problem you could ask me then.
